Step 1: Gather Required Documents

You must provide one of the following documents showing your new legal name:
  • Updated Social Security card.
  • Passport.

Tip: After completing Steps 1 and 2, review Step 3 to make sure all systems are updated.

Step 2: Follow instructions based on your role at ISU

Choose the category that applies to you:

Students

Complete the Name Change Form and submit it with your documentation:
  • In person:
    Office of the Registrar
    Museum Building

Important Notes:
  • If you are graduating, submit your name change at least one week before the last day of the term.
  • After graduation, you must pay for a new diploma if your name changes.

Faculty and Staff

After receiving your updated Social Security card:
  1. Contact Human Resources.

  2. Follow their instructions to complete the process.

Student Employees

If you are a student employee, complete both:

Note: This ensures your name is updated across all university systems and helps prevent delays.

Step 3: Update Key Records (After your Legal Name Change has been processed)

After your legal name change is processed, the following systems may require a manual update.

Bengal ID Card

You can request a replacement ID in one of two ways:
Option 1: In person
Option 2: Online
  1. Complete a BENGAL CARD ORDER (external link) form.
  2. Select Submit Form

Notes:

  • Replacement IDs are free for current students and staff after a legal name change.
  • You must show a government-issued photo ID with your new name when picking up your card.

Username / Content Management System

  1. Send an email to the IT Service Desk (external link) with the following information:
    • Current Username.
    • Preferred Username.

Systems Updated Automatically

Once your name change is processed, these systems will update automatically:
  • Chrome River (travel system).
  • Constant Contact.
  • ISU Parking System and parking passes.
  • ISU Health Center records.
  • PERSI (Public Employees Retirement System of Idaho).
  • TouchNet Marketplace and uStore.

Tip: Before your next medical appointment, contact the ISU Health Center (208)282-2330 to confirm your update has been completed.
