Step 1: Gather Legal Documentation

To update your legal name with Idaho State University (ISU), you will need to provide a copy of your updated Social Security card or passport reflecting your new legal name.

Tip: After completing Steps 1 and 2, review Step 3 to identify any systems that may require additional updates.

Step 2: Follow Instructions by Classification

(Students | Faculty & Staff | Student Employees)

Students

Complete the Name Change Form.
Submit the form with your updated documentation:
- In Person: Office of the Registrar, Museum Building, 921 S. 8th Ave, Stop 8196 Pocatello, ID 83209
- Email: reginfo@isu.edu

Note Diploma Deadline: Name change forms must be submitted at least one week before the last day of the term in which you plan to graduate. After graduation, a new diploma must be purchased if there are any name changes.

Faculty & Staff

Contact Human Resources after receiving your updated Social Security card.
HR will guide you through the name change process and any necessary forms.

Student Employees

Student employees will need to complete both the students and staff steps above to ensure their legal name is fully updated across all ISU records. This helps avoid delays and ensures everything is consistent.

Step 3: Update Key Records (After your Legal Name Change has been processed)

Students & Student Employees

After your name change has been processed, the following systems require manual updates:

Bengal ID

You can request a replacement Bengal ID using one of the following options:

Option 1: In-Person by visiting one of the following ISU locations:
- Pocatello - Campus Connection
- Idaho Falls - Information Desk
- Meridian - Front Desk
Option 2: Online:
1. Select the Bengal Card Order form link and fill out the form.
2. Click Submit Form (black button)

Notes:

Currently enrolled students and staff are eligible for a no-charge Bengal ID replacement in the event of a legal name change.
To pick up your replacement ID, present a form of government-issued photo identification that reflects your name change.

Send an email to the IT Service Desk: help@isu.edu with the following information:
- Current Username
- Preferred Username

After your legal name change request is finalized, the following systems will be updated AUTOMATICALLY:

Constant Contact
ISU Parking System and Parking Passes
- Note: Visit the ISU Parking office if you need to add a vehicle to your parking pass.
ISU Health Center - Once your name change is processed, your information will be automatically updated in ISU’s health records system.
- Note: To ensure everything is in order before your next appointment, we recommend emailing the ISU Health Center to confirm the update.

Faculty and Staff

The following systems will require you to update them MANUALLY after your legal name change request has been finalized.

Medical & Dental Insurance

Send an email to benefits@isu.edu with the following information:
- Old Legal Name
- New Legal Name
Note: Be sure your legal name change is fully finalized before updating your Medical and Dental insurance to prevent any issues with your coverage.

Life Insurance

To update your life insurance beneficiary, complete the Basic Life Insurance Beneficiary Designation Form (PDF) and return it to Human Resources.

ISU Directory Update

Click the ISU Directory Update Request link and fill out the form to update your ISU directory listing.

Bengal ID

You can request a replacement Bengal ID using one of the following options:

Complete the PCard Account Maintenance Form and work with your Administrative Assistant to get the form signatures.

ISU Health Center - Once your name change is processed, your information will be automatically updated in ISU’s health records system.
