Legally change your name with the state. You will need a social security card or passport with your new name.
  1. Fill out the ISU Name Change Form.
  2. The form can be submitted in person at the Registrar's Office, mailed, faxed, or emailed, being sure to bring or attach a copy of your passport or social security card.
