Select the Request Equipment Reservation button to access the form. Enter the required information and select Submit to complete your request.

Request instructional equipment for academic use at Idaho State University through the Instructional Technology Resource Center (ITRC).

• Equipment may be checked out for up to one week and must be picked up by the faculty or staff member who submitted the request.
• After review, you will receive an email confirming approval or denial based on availability.
