Description

Select the Request Telephone Privileges (TPR) button to access the form. Enter the required information and select Submit to complete your request.

Details

Request approval for any Telephone Move, Add, or Change that results in a new recurring charge. Approvals must be obtained from your Account Director and UBO.
  • Note: This form is required for any new service that results in a recurring charge. Telephone privilege requests must be approved by both the Account Director and the UBO.
