Employee Request for Services
Current and prospective ISU employees should follow these steps to request accommodations:
- Step 1) Fill out the request for services form by clicking the "Request Service" button to the right and provide any documentation.
- Step 2) After you have submitted the employee request, You will be contacted by our office to schedule your appointment.
- Step 3) Meet with Karina Mason Rorris, Director of Disability Services to review documentation and determine reasonable accommodations.
