Important notes:

ISU Student Housing residents should not use this form. For assistance, visit the ISU Housing Website (external link).
If your request involves relocating or modifying office space, complete the Project/Space Request (external link) form instead.

Details

Use the NetCom Work Order Request form to request repair, installation, or service of data and network communication cables, telephones, and related network infrastructure.

Networking Services

New network jack installation.
Activate existing network jack.
Network move request.
Wired network repair request.

Phone Services

New phone request (office and cellular).
- Requires a Telephone Privileges Request (TPR) (external link) Form with required approvals before NetCom Work Order submission.
Existing phone request.
Report voice or phone repair issues.
Voicemail add or remove.
Caller ID change.

Google Voice

Google Voice is available to full-time University employees for calling, texting, and voicemail. It can be used on a web browser, mobile device, or desk phone for domestic and international communication.

This service is billable and includes a departmental charge.
Requires a Telephone Privileges Request (TPR) (external link) Form with required approvals before NetCom Work Order submission.
